| Abstract: | การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาต้นทุนในมุมมองของผู้ให้บริการและคุณภาพชีวิตของผู้ป่วย ธาลัสซีเมีย หลังการรักษาด้วยการปลูกถ่ายไขกระดูกในโรงพยาบาลจุฬาลงกรณ์ โดยใช้รูปแบบการศึกษาเชิงพรรณนา แบ่งการเก็บข้อมูลเป็น 2 ส่วน ข้อมูลต้นทุนทำการเก็บข้อมูล 8 ปี (ระหว่างเดือน มิถุนายน 2542 – ธันวาคม 2549) แบ่งเป็น ต้นทุนค่าบริการทางการแพทย์ กับ ต้นทุนค่าบริการพื้นฐาน รวมกันเป็นค่าใช้จ่ายในการดูแลรักษาผู้ป่วยแต่ละราย โดยนำมาคำนวณค่าใช้จ่ายเฉลี่ย ส่วนที่สองเป็นข้อมูลคุณภาพชีวิต โดยใช้แบบสอบถามคุณภาพชีวิตเด็ก (PedsQL) สอบถามผู้ปกครองและผู้ป่วยตามช่วงอายุ แล้วนำมาคิดคะแนนเฉลี่ยของคุณภาพชีวิต ผลการศึกษาพบว่าต้นทุนเฉลี่ยของผู้ป่วยธาลัสซีเมียหลังการรักษาโดยการปลูกถ่ายไขกระดูก จำแนกตามระยะเวลา 3 เดือน, 6 เดือน และ 12 เดือน เท่ากับ 1,016,010.68, 1,055,233.38 และ 1,072,306.41 บาท/คน ตามลำดับ และถ้ารับบริจาคไขกระดูกจากญาติพี่น้องต้นทุนเฉลี่ยเท่ากับ 972,873.88, 996,963.59 และ 1,029,225.51 บาท/คน ส่วนต้นทุนที่ผู้บริจาคไม่ใช่ญาติพี่น้องเท่ากับ 1,096,121.88, 1,176,628.79 และ 1,314,046.95 บาท/คนตามลำดับ ต้นทุนเฉลี่ยของผู้ป่วยระยะเวลา 3 เดือน, 6 เดือน และ 12 เดือน จำแนกตามปริมาณเหล็กสะสม<2500ng/ml เท่ากับ 1,033,530.39, 1,076,543.02 และ 1,161,653.35 บาท/คน ตามลำดับ ถ้าปริมาณเหล็กสะสม [is more than or equal to] 2500 ng/ml เท่ากับ 975,131.34, 997,697.34 และ 957,452.20 บาท/คน ตามลำดับ ค่าเฉลี่ยคะแนนคุณภาพชีวิตของผู้ป่วยธาลัสซีเมียหลังการรักษาโดยการปลูกถ่ายไขกระดูก จำแนกตามระยะเวลา 3 เดือน, 6 เดือน และ 12 เดือน ผู้ปกครองเท่ากับ 95.36, 93.24 และ 90.22 คะแนน ส่วนของผู้ป่วย เท่ากับ 89.58, 86.95 และ 88.46 คะแนนตามลำดับ ค่าเฉลี่ยคะแนนคุณภาพชีวิตของผู้ป่วยระยะเวลา 3 เดือน, 6 เดือน และ 12 เดือน จำแนกตามความสัมพันธ์ระหว่างผู้บริจาคไขกระดูกกับผู้รับแบบญาติพี่น้อง คะแนนผู้ปกครองเท่ากับ 93.33, 95.23 และ 90.51 คะแนน ผู้ป่วยเท่ากับ 89.58, 91.41 และ 88.69 คะแนนตามลำดับ แบบไม่ใช่ญาติพี่น้องคะแนนผู้ปกครองเท่ากับ 97.40, 91.25 และ 92.51 คะแนน ด้านคะแนนผู้ป่วยระยะเวลา 6 เดือน และ 12 เดือน เท่ากับ 82.5 และ 87.88 คะแนน ค่าเฉลี่ยคะแนนคุณภาพชีวิตของผู้ป่วยระยะเวลา 3 เดือน, 6 เดือน และ 12 เดือน จำแนกตามปริมาณเหล็กสะสม<2500 ng/ml ผู้ปกครองเท่ากับ 88.33, 93.24 และ 89.40 คะแนน ด้านคะแนนผู้ป่วยระยะเวลา 6 เดือน และ 12 เดือน เท่ากับ 86.95 และ 86.54 คะแนน ปริมาณเหล็กสะสม [is more than or equal to] 2500 ระยะเวลา 3 เดือน และ 12 เดือน ผู้ปกครอง เท่ากับ 97.71 และ 96.99 คะแนน ด้านผู้ป่วยเท่ากับ 89.69 และ 93.47 คะแนน ผลการศึกษานี้สามารถใช้เป็นข้อมูลเพื่อประกอบการตัดสินใจในการให้การรักษาผู้ป่วยแก่แพทย์ |
| Other Abstract: | The purpose of this study was to analyze cost from the provider perspective and quality of life after bone marrow transplantation of thalassemia patients in King Chulalongkorn Memorial Hospital. Design : descriptive study. The cost information collected related to an 8-year period, from June 1999 – December 2006, and was categorized as medical care cost (MCC) and routine service cost (RSC). From these costs, the average cost of treatment per person was calculated. Quality of life information was collected using the PedsQL questionnaire instrument for specific adult (parent) and child (patient) age groups. Questionnaire results were used to calculate average quality of life scores. The study revealed that the average cost of treatment of thalassemia patients after bone marrow transplantation at 3, 6 and 12 months were 1,016,010.68, 1,055,233.38 and 1,072,306.41 Thai Baht (THB) per person respectively, and that where sibling donors were used, the average cost of treatment was 972,873.88, 996,963.59 and 1,029,225.51 THB per person. Unrelated donors represented a cost of 1,096,121.88, 1,176,628.79 and 1,314,046.95 THB per person. The average cost of treatment was also analyzed by patient serum ferritin results. When ferritin levels were <2500 ng/ml the costs were 1,033,530.39, 1,076,543.02 and 1,161.653.35 THB per person at 3, 6 and 12 months respectively. When ferritin levels were [is more than or equal to] 2500 ng/ml the costs were 975,131.34, 997,697.34 and 957,452.20 THB per person at the same time intervals. The quality of life score after bone marrow transplantation after 3, 6 and 12 months were 95.36, 93.24 and 90.22 respectively for parents and 89.58, 86.95 and 88.46 for patients. By comparison, were this involved a sibling donor, quality of life scores were 93.33, 95.23 and 90.51 for parents and 89.58, 91.41 and 88.69 for patients. Where this involved an unrelated donor, scores were 97.40, 91.25 and 92.51 for parents and 82.50 and 87.88 (at 6 and 12 months) for patients. The quality of life score was also analyzed by serum ferritin results. When ferritin levels were < 2500 ng/ml, scores were 88.33, 93.24 and 89.40 (at 3, 6 and 12 months respectively) for parents and 86.95 and 86.54 (at 6 and 12 months) for patients. When ferritin levels were [is more than or equal to] 2500 ng/ml, scores were 97.71 and 96.99 (at 3 and 12 months) for parents and 89.69 and 93.47 (at 3 and 12 months) for patients. These results could be applied as a basis for deciding the treatment of thalassemia patients by physicians. |
