| Abstract: | ศึกษาต้นทุนกิจกรรมการพยาบาทางตรงตามกระบวนการหลัก 3 กิจกรรมประกอบด้วย กิจกรรมการพยาบาลระยะก่อนตรวจ กิจกรรมการพยาบาลระยะตรวจ กิจกรรมการพยาบาลระยะหลังตรวจ และต้นทุนกิจกรรมการพยาบาลทางอ้อมตามกระบวนการหลัก 2 กิจกรรม ประกอบด้วย กิจกรรมการพยาบาลระยะก่อนตรวจ และกิจกรรมการพยาบาลระยะหลังตรวจ ในห้องตรวจโรคเด็ก งานกุมารเวชกรรม โรงพยาบาลตำรวจ จำแนกเป็น 41 กิจกรรมย่อย วิเคราะห์โดยใช้ระบบการคิดต้นทุนกิจกรรม (ABC) กลุ่มตัวอย่างคือ บุคลากรพยาบาลที่ปฏิบัติงานในห้องตรวจโรคเด็ก งานกุมารเวชกรรม โรงพยาบาลตำรวจ จำนวน 9 คน ระยะเวลาที่ศึกษาคือ ตั้งแต่ 1 พฤศจิกายน 2546 ถึง 31 มกราคม 2547 เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลคือ พจนานุกรมกิจกรรมการพยาบาลและแบบบันทึกข้อมูลจำนวน 7 ชุด ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ และตรวจสอบความเที่ยงของแบบสังเกตได้ค่าความเที่ยงเท่ากับ .87 ผลการศึกษาที่สำคัญสรุปได้ดังนี้ 1. ค่าเฉลี่ยของต้นทุนต่อหน่วยกิจกรรมการพยาบาลในห้องตรวจโรคเด็ก ตั้งแต่ระยะก่อนตรวจถึงระยะหลังตรวจ มีค่าเท่ากับ 27,543.55 บาท 2. ค่าเฉลี่ยของต้นทุนต่อหน่วยกิจกรรมการพยาบาลทางตรง ในห้องตรวจโรคเด็ก ตั้งแต่ระยะก่อนตรวจถึงระยะหลังตรวจ มีค่าเท่ากับ 26,673.49 บาท 3. ค่าเฉลี่ยของต้นทุนต่อหน่วยกิจกรรมการพยาบาลทางอ้อม ในห้องตรวจโรคเด็ก ตั้งแต่ระยะก่อนตรวจถึงระยะหลังตรวจ มีค่าเท่ากับ 870.06 บาท 4. ค่าเฉลี่ยต้นทุนกิจกรรมการพยาบาลทางตรงในกิจกรรมย่อย ที่มีต้นทุนต่อหน่วยกิจกรรมสูงสุด ได้แก่ กิจกรรมพยาบาลในการใส่สาย Foley's catheter เพื่อทำ Voiding Cysto Utero Graphy เท่ากับ 8,595.39 บาท รองลงมาได้แก่ กิจกรรมพยาบาลการให้สารน้ำ (IV Fluid) เท่ากับ 6,804.68 บาท และกิจกรรมพยาบาลในการใส่ NG Tube เพื่อทำ Gastric wash เท่ากับ 4,950.32 บาท ส่วนกิจกรรมการพยาบาลย่อยที่มีต้นทุนต่อหน่วยกิจกรรมเฉลี่ยต่ำที่สุด ได้แก่ กิจกรรมพยาบาลการจัดลำดับผู้ป่วยเด็กเข้ารับการตรวจ และกิจกรรมพยาบาลการจัดผู้ป่วยเด็กเข้ารับการตรวจตามสภาวะสุขภาพ และ/หรือตามลำดับเท่ากับ 0.29 บาท 5. ค่าเฉลี่ยต้นทุนกิจกรรมการพยาบาลทางอ้อมในกิจกรรมย่อย ที่มีต้นทุนต่อหน่วยกิจกรรมสูงสุด ได้แก่ กิจกรรมการเตรียมความพร้อมของสถานที่ เท่ากับ 611.49 บาท รองลงมา ได้แก่ กิจกรรมการเตรียมความพร้อมของวัสดุอุปกรณ์ เท่ากับ 182.27 บาท ส่วนกิจกรรมการพยาบาลทางอ้อมในกิจกรรมย่อย ที่มีต้นทุนต่อหน่วยกิจกรรมเฉลี่ยต่ำที่สุด ได้แก่ กิจกรรมการจัดเตรียมและตรวจสอบเอกสาร เท่ากับ 0.47 บาท |
| Other Abstract: | To analyze the costs of direct and indirect nursing activities of the out-patient Pediatric Department, the Police General Hospital during pre examination to post examination according to significant caring process of total out-patient Pediatric Department using activity-based costing system analysis. The direct activities to be analyzed consisted of 3 major activities, including pre examination process, during examination process, post examination process. The indirect activities to be analyze consisted of 2 major activities, including pre examination process and post examination process. They were devided into 41 sub-activities. The samples were 9 nursing personnel working in the out-patient Pediatric Department, the Police General Hospital. Data were collected during November 1, 2003 to January 31, 2004. Research instruments composed of 7 nursing activity record forms. All instruments were tested for content validity and reliability. The reliability of observation was .87 The major results of this studywere as follows: 1. Mean cost of all nursing activities in the out-patient Pediatric Department during pre examination to post examination was ฿27,543.55. 2. Mean cost of direct nursing activities in the out-patient Pediatric Department was ฿26,673.49. 3. Mean cost of indirect nursing activities in the out-patient Pediatric Department was ฿870.06. 4. The highest mean cost of sub direct nursing activities was retaining foley's catheter for Voiding Cysto Utero Graphy, was ฿8,595.39, taking intravascular fluid was ฿6,804.68, and taking nasogastric tube was ฿4,950.32, the lowest cost was piority patient to examination activities was ฿0.29. 5. The highest mean cost of sub indirect nursing activities was preparing area was ฿611.49, preparing instrument was ฿182.27, the lowest cost was preparing and checking paper was ฿0.47. |
