| Abstract: | วัตถุประสงค์: ศึกษา ผลของการระงับปวด อาการปัสสาวะลำบาก ตลอดจน ผลข้างเคียงต่าง ๆ ภายหลัง การผ่าตัด ริดสีดวงทวารหนักแบบเย็บแผลปิด เปรียบเทียบระหว่าง วิธีฉีดยาชาเฉพาะที่รอบทวารหนัก กับการฉีดยาชาเข้าช่องไขสันหลัง รูปแบบการวิจัย : การวิจัยเชิงทดลองแบบสุ่ม สถานที่ทำวิจัย : โรงพยาบาลพระมงกุฎเกล้า ระเบียบวิธีวิจัย: ผู้ป่วยจำนวน 64 รายที่เข้ารับการผ่าตัดริดสีดวงทวารหนัก เป็นเพศชาย 32 ราย เพศหญิง 32 ราย ได้รับการสุ่มเพื่อให้ยาชาระงับปวดแบบฉีดยาชาเข้าช่องไขสันหลัง (SA) 32 ราย และฉีดยาชาเฉพาะที่รอบทวารหนัก(LA) 32 ราย ทำการเก็บข้อมูลของ ระยะเวลาที่เริ่มมีอากาปวดแผล วัดระดับของอาการปวดแผนโดย visual analogue scale (VAS) ที่เวลา 6 และ 24 ชั่วโมงหลักผ่าตัด ปริมาณยาแก้ปวดที่ได้รับภายใน 24 ชั่วโมง หลังผ่าตัด ภาวะแทรกซ้อนต่าง ๆ และความพึงพอใจของผู้ป่วยต่อวิธีการให้ยาชาระงับปวด ผลการศึกษา : ระดับอาการปวดเปรียบเทียบระหว่าง วิธีฉีดยาชาเฉพาะที่รอบทวารหนัก (LA) กับ การฉีดยาชาเข้าช่องไขสันหลัง (SA) ที่เวลา 6 ชั่วโมง (AS: 51.9 ± 28.9 vs. LA: 41.7 ± 23.5; P = 0.13) และ 24 ชั่วโมง (AS: 37.6 ± 19.3 vs. LA: 33.9 ± 21.2; P = 0.47) หลังผ่าตัดไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ กลุ่มผู้ป่วยที่ได้รับการการฉีดยาชาเข้าช่องไขสันหลังเรียกหายาฉีดระงับปวดหลังการผ่าตัด มากกว่า กลุ่มผู้ป่วยที่ได้รับการฉีดยาชาเฉพาะที่ (P = 0.035) และ มีปัญหาการปัสสาวะลำบาก มากกว่ากลุ่มผู้ป่วยที่ได้รับการฉีดชายาเฉพาะที่รอบทวารหนักอย่างมีนัยสำคัญทางสถิติ (SA: 62.5% vs. LA: 28.1%; P = 0.011) สรุป: การผ่าตัดริดสีดวงทวารหนักโดยการฉีดยาชาระงับปวดเฉพาะที่ สามารถลดอาการปวดหลังผ่าตัดได้ดี ไม่แตกต่างจากการฉีดยาชาเข้าช่องไขสันหลัง อีกทั้ง ช่วยลดปัญหาปัสสาวะลำบาก และลดปริมาณการใช้ยาฉีดแก้ปวดภายหลังการผ่าตัดลงได้ |
| Other Abstract: | Objectives: To study analgesic efficacy, postoperative voiding problems, patients’ satisfaction, and other complications after closed hemorrhoidectomy comparison between local perianal block and spinal anesthesia. Research design: Randomized controlled trial Setting: Phramongkutklao Hospital Research methodology: A total of 64 subjects (32 males and 32 females) underwent elective hemorrhoidectomy were randomly allocated into two groups. Thirty-two patients were randomly allocated to receive spinal anesthesia (AS group) while 32 patients received local perianal block (LA group). Duration of analgesic effect, pain measurement with visual analogue scale (VAS) at 6 and 24 hours, quantity of analgesic medication administered, postoperative complication, and patient’s satisfaction with the anesthetic technique were recorded. Result: There was no significant difference in the degree of pain at 6 hours (AS: 51.9 ± 28.9 vs. LA: 41.7 ± 23.5; P = 0.13) and at 24 hours (SA: 37.6 ± 19.3 vs. LA: 33.9 ± 21.2; P = 0.47) after surgery between the two groups. Patients with spinal anesthesia required more analgesics injection than patients with local perianal block (P = 0.035). There was higher rate of voiding problems in spinal anesthesia than local anesthesia (SA: 62.5% vs. LA: 28.1%; P = 0.011). Conclusion: Hemorrhoidectomy under local perianal block was feasible and safe. Postoperative pain intensity did not difference to spinal anesthesia. However, patients with local anesthesia required less analgesic injections and had fewer postoperative voiding complications. |
