| Abstract: | การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อวิเคราะห์ต้นทุน-ประสิทธิผลในการรักษาผู้ป่วยโรคเอดส์ระยะมีอาการสัมพันธ์กับเอดส์ โดยการเปรียบเทียบต้นทุน-ประสิทธิผลในการรักษาผู้ป่วยที่ได้รับยา Zidovudine กับผู้ป่วยที่ไม่ได้รับยานี้ การศึกษาครั้งนี้เป็นการศึกษาเฉพาะกรณี โรงพยาบาลจุฬาลงกรณ์ประชากรตัว อย่างที่ใช้ในการวิจัย แบ่งออกเป็น 2 กลุ่ม คือกลุ่มที่ได้รับการรักษาด้วยยา Zidovudine จำนวน 66 รายและกลุ่มที่ไม่ได้รับยานี้จำนวน 44 ราย การคัดเลือกประชากรเข้ามาศึกษาใช้วิธีการจับคู่เพื่อเลือกตัวอย่างเข้ามาศึกษาตามเกณฑ์ที่กำหนดไว้ ผลการวิจัยพบว่า ต้นทุนรวมในการรักษาผู้ป่วยกลุ่มที่ได้รับยา Zidovudine เฉลี่ยต่อรายปีเท่ากับ 111, 729 บาท โดยแบ่งเป็นต้นทุนที่เกิดกับผู้จัดบริการ 1,234 บาท (ร้อยละ 1.10) ต้นทุนที่เกิดกับผู้รับบริการ 110,495 บาท (ร้อยละ 98.90) ในส่วนของต้นทุนที่เกิดกับผู้รับบริการนั้นเป็นต้นทุนค่ายา Zidovudine 69,300 บาท (ร้อยละ 62.72) ค่ายาทั่วไป 3,244 บาท (ร้อยละ 2.94) ค่าตรวจทางห้องปฏิบัติการ 9,534 บาท (ร้อยละ 8.63) ค่ารังสีวินิจฉัย 768 บาท (ร้อยละ 0.69) นอกนั้นเป็นต้นทุนอื่น ๆ อีก 27,647 บาท (ร้อยละ 25.02) ต้นทุนรวมในการรักษาผู้ป่วยกลุ่มที่ไม่ได้รับยา Zidovudine เท่ากับ 52,105 บาท เป็นต้นทุนที่เกิดกับผู้จัดบริการ 1,591 บาท (ร้อยละ 3.05) ต้นทุนที่เกิดกับผู้รับบริการ 50,513 บาท (ร้อยละ 96.95) ต้นทุนที่เกิดกับผู้รับบริการแบ่งเป็นค่ายาทั่วไป 3,491 บาท (ร้อยละ 6.77) ค่าตรวจทางห้องปฏิบัติการ 10,119 บาท (ร้อยละ 20.03) ค่ารังสีวินิจฉัย 1,324 บาท (ร้อยละ 2.62) นอกนั้นเป็นต้นทุนอื่น ๆ อีก 35,758 บาท (ร้อยละ 70.43) สำหรับการวิเคราะห์ต้นทุน-ประสิทธิผลในการรักษาผู้ป่วยด้วยยา Zidovudine นั้นปรากฏว่า การใช้ ยา Zidovudine นั้นมีความน่าจะเป็นในการชะลออัตราความก้าวหน้าของโรคลงได้ร้อยละ 65.2 โดยมีสัด ส่วนของต้นทุนในการชะลอความก้าวหน้าของโรคเท่ากับ 1,713 บาทต่อหนึ่งหน่วย ประสิทธิผล จำนวนปีชีวิตผู้ป่วยที่ประหยัดได้เมื่อผู้ป่วยได้รับยา 1 ปี เท่ากับ 16.06 คน-ปีสัดส่วนของต้นทุนต่อจำนวน 1 คน-ปีเท่ากับ 459,162 บาท จำนวนปีของผู้ชีวิตผู้ป่วยที่ประหยัดได้เมื่อผู้ป่วยได้รับยาต่อเนื่องเท่ากับ 140.07 คน-ปี สัดส่วนต้นทุนต่อ 1 คน-ปีเท่ากับ 671,357 บาท จำนวนปีที่ผู้ป่วยมีอายุยืนยาวขึ้นเมื่อได้รับยาหนึ่งปีเท่ากับ 0.24 ปี เมื่อผู้ป่วยได้รับยาต่อเนื่องจะมีอายุยืนยาวขึ้น 2.13 ปีในการลดอัตราการเป็นโรคฉวยโอกาสนั้นเมื่อเปรียบเทียบกับกลุ่มที่ไม่ได้รับยา มีความน่าจะเป็นในการลดอัตราการเป็นโรคแทรกลงได้ร้อยละ 27.4 จากผลการศึกษาสรุปได้ว่า การใช้ยา Zidovudine ในผู้ป่วยโรคเอดส์ระยะมีอาการสัมพันธ์กับเดอส์จะต้องใช้ค่าใช้จ่ายที่สูงมาก โดยเฉพาะค่ายา ซึ่งรัฐไม่สามารถรับภาระค่าใช้จ่ายส่วนนี้ได้ ดังนั้นการรักษาด้วยยานี้ควรคำนึงถึงความสามารถในการจ่ายของผู้ป่วยและความพอใจที่จะจ่ายของผู้ป่วย เมื่อคำนึงถึงผลที่ได้รับจากการใช้ยาดังกล่าวแล้ว |
| Other Abstract: | The purpose of this thesis was to compare the cost-effectiveness of treating AIDS patients with or without zidovudine. Study was conducted at Chulalongkorn Hospital. The samples of 110 HIV positive patients was divided into two groups; a study group of 66 patients who received zidovudine and a control group of 44 patients who did no receive zidovudine. Patients were assigned to the study or control group based upon matching criteria. The result of the study showed that the annual cost per case of zidovudine treatment group was Baht 111,729. The cost incurred by the provider was Baht 1,234 (1.10%), and the cost incurred each consumer was Baht 110,495 (98.0%) The average costs incurred by each consumer were zidovudine drug, Baht 69,300 (62.72%), other drugs Baht 3,244 (2.94%), laboratory cost, Baht 9,534 (8.63%), X-rays cost, Baht 768 (0.69%) and other cost, Baht 27,647 (25.02%). The annual cost, per case for patients in the control group without zidovudine treatment was Baht 52,105. The cost incurred by the provider was Baht 1,591 (3.05%), and the average cost incurred by each consumer was Baht 50,513 (96.95%). The average cost incurred by each consumer was drugs, Baht 3,491 (6.77%) laboratory cost, Baht 10,119 (20.03%), X-rays cost, Baht 1,324 (2.62%) and others cost, 35,578 (70.43%). In terms of percentage effectiveness, zidovudine therapy help to delay the onset of AIDS in the ARC group by 65.2%. One unit of effectiveness in delaying the progression of the HIV diseases cost Baht 1,713. The years of life saved in the treated group for one year of treatment was 16.06 man-year. The cost per one man-year was Baht 459,162. The years of life saved when all the patients on the study group receive continuous treatment with zidovudine drug was 140.70 man-year. The cost per one man-year life saved in the study group was Baht 671,357. The life years gain when each patients received the drug for one year was 0.24 years and 2.13 man-year when is receipt of continuous treatment. Zidovudine grug decreased the opportunistic infection rate by 27.4%. The study can be conducted that the use of zidovudine therapy in AIDS patient is extremely expensive, particularly the cost of drugs. It is very unlikely that the government can afford to provide zidovudine therapy without user charge. Therefore, the zidovudine therapy may be provided to patient who has ability and willingness to pay under full information of the level of zidovudine effectiveness. |
